Hosted Form Example

This is a live example of a hosted form. Instead of embedding the form into a website, SingleForm hosts a branded landing page for you at f.singleform.ai/{shortCode}. Share the link anywhere — email, SMS, print QR codes, social posts — and visitors land on a page that walks them through scanning with the SingleForm app.

On desktop, the page shows a QR code — scan with the SingleForm app. On mobile, tap Open in SingleForm to launch the app via universal link.

How It Works

  1. Visitor opens the link — they land on a branded page showing the form title, your business name, and a verified domain badge
  2. Secure session starts — clicking “Start Secure Session” opens a unique, one-time connection for that visit
  3. QR or deep link — desktop visitors see a QR with a 3-minute countdown; mobile visitors get an “Open in SingleForm” button
  4. Submit from phone — the user scans/taps, fills the form in the SingleForm app, and submits
  5. Live feedback — the landing page updates in real time with success or failure as soon as the webhook delivery completes
